8. Each year, at the same time you update your child's picture, record the current information as of the posted date. Keep this information current.

MEDICAL RECORD:

Include the following medical information on page three of your CHILD IDENTIFICATION KIT:

- Hair color
- Eye color
- Blood type
- Whether their ears are pierced
- Medical conditions
- Allergies
- Scars location identification
- Birthmark location identification
- Mole location identification
- Any other unique identification features on your child

DENTAL RECORDS:

1. Obtain a complete set of dental x-rays from your child's dentist and place them in your CHILD IDENTIFICATION KIT.

2. Authorities can often identify a person from their dental records.

On the back portion of your CHILD INFORMATION KIT record the following telephone numbers:

- Police department
- Fire department
- Nearest three hospitals
- Daycare center
- Poison control center
- Other important telephone numbers
